4. You arrange with our office a time convenient to you when an Executive Analyst will be on premises to conduct the survey of your business operations. Surveys begin on Mondays and Thursdays.





5. You, and a majority of the business owners, meet with the Executive Analyst before and after the survey is conducted to share your business concerns, business objectives, survey results and recommendations for improvement.

Following is the criteria for a business to be eligible for the Business Survey Service. The W-2 (or the Canadian equivalent government form) employee count includes both full and/or part-time. Sales volume is the gross sales dollar (US Dollar or Canadian Dollar if applicable) amount earned within the past 12 months.

  • your business is not a publicly traded corporation (it must be a privately owned business) and/or you have complete autonomy in how you operate the business (an auto dealership for example).

  • your business is not a susidiary owned by a parent company.

  • your business is a for-profit company (ie: not a non-profit, association or religious organization).

  • your business is not a franchise.

  • your business is not a home-based business.

  • In General:


    • United States, minimum qualifications (US dollars) are:


      • For most companies:
        • at least 7 W-2 employees and at least $500,000 sales volume in the past 12 months or

        • any number of W-2 employees and at least $1 million sales volume in the past 12 months.


      • For restaurants, bars, lounges: at least 10 W-2 employees and at least $750,000 sales volume in the past 12 months.


    • Canada, minimum qualifications (Canadian dollars) are:


      • For most companies:
        • at least 7 W-2 employees and at least $750,000 sales volume in the past 12 months or

        • any number of W-2 employees and at least $1.5 million sales volume in the past 12 months.


      • For restaurants, bars, lounges: at least 10 W-2 employees and at least $1 million sales volume in the past 12 months.



  • Exceptions:


    • We do not provide consulting services to gasoline stations without convenience store/food operations.

    • For gasoline stations with convenience store/food operations, consider only the convenience store/food operations sales volume (not gasoline sales), which must be at least $500,000 if located in the United States, or at least $1 million if located in Canada. The operation must have at least 7 W-2 employees.

    • Convenience stores that do not sell gasoline must have at least 7 W-2 employees and at least $500,000 sales volume if located in the United States, or at least $1 million if located in Canada.

    • Bowling alleys, car washes and IGA food stores must fit the following criteria:


      • Independently owned, not a franchise or chain.

      • At least 7 W-2 employees.

      • At least $1 million sales volume if in the United States.

      • At least $1.5 million sales volume if in Canada.


    • Hotels and motels must fit the following criteria:


      • Independently owned, not a franchise or chain.

      • At least 7 W-2 employees.

      • At least $500,000 sales volume if in the United States.

      • At least $750,000 sales volume if in Canada.



  • For statistical purposes the dollar volume of certain types of companies will be based on the following:


    • Commissions for Insurance Companies.

    • Commissions for Brokers (any type).

    • Commissions for Travel Agencies.

    • Commissions for Auction Companies.

    • Commissions for Manufacturer's Representatives.

    • Commissions for Real Estate Agencies.

Business Survey Authorization Form 68-A

Here are instructions on how to complete Form 68-A and, provided your business qualifies for the survey service, how to initiate a survey of your business.

Here are steps you and I must take to initiate a business survey for your business:

1. You: Print and complete the Business Survey Authorization Form 68-A:

  • Write the name, address and contact information for your business at the top of the form.

  • Write the date, on a Monday or a Thursday, and time you want to meet with the Executive Analyst, where it says "to make a preliminary study of our company beginning_______________".

  • Sign the form, on the line above "Authorized By", and complete the business details section below it including the number of W-2 employees full and part time and the approximate gross dollar sales in the past 12 months.


2. You: Fax the completed form to my (Mark Bruemmer's) attention at 270-738-4587. Or you could scan the completed form and email it to me at GMAYUSASE@gmail.com as an attachment.

3. Me: I'll review the form and contact you if I have any questions. I'll contact GSMIC Corporate Office and provide them the information you've given me.

4. GSMIC: A Representative in Survey Service contacts you to confirm details of your business and the date, time and place you agree to meet with the Executive Analyst.

5. You: Prepare for the business survey by writing down any business problems and goals you have for your company.  The more information the Executive Analyst has regarding your business challenges and your plans for the future, the greater the value and benefit you'll receive in return.
